[logback-user] Log IP address.

Hi Thomas,

The conversion specifier %h is logback-access specific and unavailable 
in logback-classic. For the list of conversion words available in 
logback-classic see

   https://logback.qos.ch/manual/layouts.html#conversionWord

You should place the remote host address in MDC as you did for xForwaredFor.
